How to Signup for Memorial MyChart

Use your activation code to sign up for Memorial MyChart.

Your activation code, the key to getting access to your MyChart health information, is located on your Discharge Summary, provided to you at:

• All Memorial Physicians Group (MPG) offices
• South Broward Community Health Services
• Inpatient hospital stay
• Outpatient visits to the Operating Room, Endoscopy or the Memorial Cardiac and Vascular Institute
• Emergency Room and Urgent Care visits

Activation codes are provided to adult and teen patients. Instructions about obtaining proxy access to the medical records of a child or adult are included in Discharge Summaries.
