What is Memorial MyChart?

Memorial MyChart is a free service that provides Memorial Healthcare Systems patients with direct online access to portions of their electronic health records (EHR). Information entered by providers, including laboratory results, problem lists, allergies, medications, immunizations and medical history are all securely stored for quick retrieval. How do I sign up?

You can obtain a MyChart activation code from your participating MyChart doctor's office, on your hospital discharge paperwork, or from the Memorial Healthcare System Health Information Management (HIM) Department. You can then use the activation code to create a user ID and password on the website.

What can I expect to see for test results in Memorial MyChart?

When can I see my test results in Memorial MyChart? Your test results are usually released to your Memorial MyChart account within three(3)business days. Certain restrictions apply. What kind of test results can I expect to see in Memorial MyChart? You will be able to receive most laboratory and radiology results. Test results for sexually transmitted disease, HIV, and alcohol and substance abuse will not be automatically released to Memorial MyChart. Will my test results in Memorial MyChart include any notes or explanations from my doctor? Your participating Memorial MyChart doctor may choose to write a note to accompany your test result in Memorial MyChart.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record. To correct any inaccurate information, please contact your doctor's(MyChart Provider's) office regarding your records or Memorial Healthcare System Health Information Management(HIM) Department regarding your hospital records. Your health information is reviewed and updated in your electronic medical record during each visit.

What types of messages can I send to my participating Memorial MyChart doctor's office through Memorial MyChart?

MyChart messaging offers you the convenience of requesting appointments, renewing prescriptions and communicating non-urgent matters.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 5 business days. Please note that MyChart should not be used for urgent situations. Please contact your provider's office if the situation requires immediate attention or dial 911 if it is an emergency.

Can I view a family member's health record in MyChart?

Yes you can. This is called Proxy access and allows a parent, guardian or designee to log into his or her own personal MyChart account, and then connect to information regarding the family member. Proxy access can be used to view health records for minor children and adult patients. Complete the appropriate Proxy/Authorization form(s) and returun it to your participating Mychart doctor's office or Memorial Healthcare System's Health Information Management(HIM) Department within the hospital to request access to this convenient service. You can obtain the proxy consent forms in the upper left corner of the screen under "Proxy Consent Forms".

MyChart for a teen, aged 13-17, has a limited view of patient health information due to Florida Statute requirements regarding teen confidentiality. This is known as "Teen View" and includes allergies, immunizations, messaging, and appointments. On your teen's 18th birthday, the Parent Proxy view will be automatically terminated. Return to Top

Can I ask questions regarding a family member from my Memorial MyChart account?

Memorial MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one Memorial MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit proxy consent forms and establish his or her own Memorial MyChart account.Please click on the link in the upper left corner of the Memorial MyChart home page to download the proxy forms.

I forgot my password. What should I do?

You may contact your physician's (MyChart Provider's)office if you are unable to reset your password by clicking on the "Forgot Password" link on the sign-in page. If you do not have a participating Memorial MyChart doctor, you may call the Memorial MyChart Help Desk at 954-276-7676 or email MyChart@mhs.net.

Can you send me a new access code as I have lost it, let it expire or did not receive it?

To obtain a new access code, you must request one in person at your participating Memorial MyChart Provider's office or the Memorial Healthcare System Health Information Management(HIM) department.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls his or her password,and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

Memorial MyChart is owned and operated by MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.

I was logged out of Memorial MyChart, what happened?

We aim to protect your privacy and security of your information. While logged into Memorial MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of Memorial MyChart. We recommend that you log out of your Memorial MyChart session if you need to leave your computer for even a short period of time.

What do I need to use Memorial MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer or Mozilla Firefox).

My access code does not work, what should I do?

For your security, your access code expires after 60 days and is no longer valid after the first time you use it. If your code has expired, contact your physician's (MyChart Provider's) office or Memorial Healthcare System Health Information Management(HIM) department to obtain a new access code. If you still have problems logging in, call your participating Memorial MyChart doctor's office or the Memorial MyChart Help Desk at 954-276-7676 .

Is my access code my user ID?

No, your access code is not your Memorial MyChart ID or password. You will use this code only once to log into Memorial MyChart for the first time. (The code will expire after you have used it or after 60 days). When you log into Memorial MyChart the first time, you will then be asked to create your own unique Memorial MyChart ID and password.

Can I access Memorial MyChart through a mobile device?

Yes, you can access Memorial MyChart through an iPhone or Android device. The mobile application has most, but not all of the Memorial MyChart functionality. For more information, visit:
